How to make a two column form

Go to the Fast Secure Contact Form settings page, select the form you want to edit and click on the Basic Settings tab.

Put this HTML in the “Welcome Introduction” setting on the basic settings tab.

Click on the Advanced tab.
Put this HTML in the “After form additional HTML” setting on the Advanced tab.

Click on the Fields tab.
Locate a field half way down your fields list on the Fields tab.
(note: because of the CAPTCHA field and the submit button, you might want to select a field even lower down the list than you might normally expect)
Click the ‘Show Details’ button for the field you selected
Put this HTML in the “HTML after form field” in the field settings.

Click the “Save Changes” button
View the form to see the results.

Move the HTML in the “HTML after form field” to a different field as needed.
You can adjust the table style as needed in step 1.
